This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure system.
Thank you for your request to enroll your student in the Lake Mills Area School District. Complete the required fields to request an account for an Enrollment Application. If you are completing enrollment for multiple children, you will need to complete one Enrollment Application for EACH child.
